• Two (2) most recent pay stubs: These should reflect your current earnings and be dated within the last 30 days.

• Alternatively, if you are self-employed or do not receive pay stubs, you may submit a recent official hire letter from your employer outlining your employment status, job title, and current salary or hourly wage in place of pay stubs.

2. Identification

• A clear and legible copy of a government-issued photo identification card or driver’s license. This is required to verify your identity.
